| Indication restriction of reimbursement ? | Fixní kombinace pertuzumabu a trastuzumabu je hrazena: 1) v adjuvantní léčbě, pokud je podávána v kombinaci s taxanovou chemoterapií (v rámci adjuvantního chemoterapeutického režimu) u dospělých pacientů s HER2-pozitivním časným karcinomem prsu s vysokým rizikem rekurence a s postiženými mízními uzlinami. Jedná se pacienty dosud neléčené pro karcinom prsu předoperační chemoterapií, ve stavu výkonnosti ECOG 0-1. Terapie je hrazena do rekurence onemocnění či do vyčerpání maxima 18 cyklů podaných v průběhu 1 roku nebo nepřijatelné toxicity (dle toho, co nastane dříve). 2) v kombinaci s docetaxelem u dospělých pacientů s HER2-pozitivním metastazujícím nebo lokálně rekurentním neresekovatelným karcinomem prsu, kteří dosud nebyli léčeni anti-HER2 léky nebo chemoterapií pro metastatické onemocnění. Všichni léčení pacienti musí mít validní laboratorní metodou v referenční laboratoři prokázanou HER-2 pozitivitu - IHC 3+ nebo ISH+. Pacienti musí mít výkonnostní stav 0 - 1 dle ECOG, nesmějí jevit klinické známky svědčící o přítomnosti mozkových metastáz a zároveň hodnota ejekční frakce levé komory musí dosahovat alespoň 50 %. Po ukončení podávání docetaxelu je terapie fixní kombinací pertuzumabu a trastuzumabu hrazena do progrese onemocnění. |
